java-decompiler: fixes and cleanups

- console decompiler: resource closing, lookup instead of scan, error reporting
- logger interface reworked
- saver interface renamed
- bytecode provider returns byte array (to reduce stream leakage)
- extra level of context unit avoided
- unneeded exceptions, dead code, formatting
